Title: Standard of Filing Fees for The National Health Insurance Certificate CH

Article 1
This Standard is stipulated in accordance to subparagraph 3, article 7 and article 10 of the Charges and Fees Act.
Article 2
The filing fee for every National Health Insurance Certificate (hereinafter referred to as NHI IC Card) by the insured from the Insurer is NT$200. However, first time applicants, as well as card renewal applications due to one of the following reasons are free of charge:
1. Card disabled by non-artificial bending, scratching, cracking,or deforming of the exterior and the IC chip;or abrasion,discoloring, or falling off of the IC chip.
2. Misprinting of the insured’s basic information on the card by the Bureau.
Article 3
This Standard is in effect on the date of its promulgation.
